Key Responsibilities: Conduct client assessments to identify needs, risks, and strengths. Develop and implement short-term care plans in collaboration with clients and multidisciplinary teams. Provide crisis intervention, counseling, and emotional support. Connect clients with community resources (e.g., housing, healthcare, financial assistance). Qualifications & Requirements: Education: Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited program. Licensure: Current state social work license (LBSW, LMSW, LCSW, or equivalent) as required. Experience: 2+ years in social work, case management, or a related field. Experience in child welfare, healthcare, or crisis intervention is a plus. Skills: Strong interpersonal and communication skills. Ability to work independently and manage a caseload efficiently. Knowledge of community resources and social service systems. Cultural competency and trauma-informed practice. Other: Valid driver’s license, clean background check, and ability to travel locally if needed.
