This role involves assessing client needs, developing intervention plans, connecting clients with resources, and advocating for vulnerable populations. The ideal candidate will be adaptable, compassionate, and skilled in crisis intervention while maintaining compliance with agency policies and legal regulations. Key Responsibilities: Conduct client assessments to identify needs, risks, and strengths. Develop and implement short-term care plans in collaboration with clients and multidisciplinary teams. Provide crisis intervention, counseling, and emotional support. Connect clients with community resources (e.g., housing, healthcare, financial assistance). Maintain accurate and confidential case records in compliance with legal and agency standards. Advocate for clients’ rights and access to services. Collaborate with schools, healthcare providers, courts, or other agencies as needed. Participate in case conferences, staff meetings, and training sessions. Ensure smooth transition of cases to permanent social workers or follow-up services. Qualifications & Requirements: Education: Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited program. Licensure: Current state social work license (LBSW, LMSW, LCSW, or equivalent) as required. Experience: 2+ years in social work, case management, or a related field. Experience in child welfare, healthcare, or crisis intervention is a plus. Skills: Strong interpersonal and communication skills. Ability to work independently and manage a caseload efficiently. Knowledge of community resources and social service systems. Cultural competency and trauma-informed practice. Other: Valid driver’s license, clean background check, and ability to travel locally if needed. Work Environment: Settings may include hospitals, schools, government agencies, or nonprofit organizations. Hybrid/remote options possible depending on assignment.
